Customize the Online Consent Form

You can add your organization's logo or banner to the top of the form (Bundle 2 and higher).

Step 1:

Step 2: Select Portal Customization menu

Step 3: Delete the default logo (Protect Youth Sports) and upload your organization logo or banner.

Logo Image - The suggested Logo size is 300 x 300 pixels
Banner Image - The suggested Banner size is 720x 150

Step 4: For Bundle 4 customers only, add Home Page URL, Introduction and Announcements (these are all optional)

Step 5:  Preview your changes when finished by clicking the Update and Preview button.
